This policy covers this website. It is not a HIPAA Notice of Privacy Practices, which is a separate document covering your dental records and is available from the office.
What This Website Collects
If you send the contact form, we receive the name, phone number, email address, the service you selected and anything you write in the message box, along with the page you sent it from. That is the whole of it. We do not ask for a date of birth, an insurance member ID or any clinical detail, and the form asks you not to include medical history.
The site also records ordinary technical information such as your IP address, which is used to filter automated spam submissions.
What It Is Used For
Form submissions are used to contact you about your enquiry and to book your appointment. Submissions are stored on this website and are also sent to a lead-management system operated by McWilliams Media, the agency that maintains this site, so that enquiries are not lost.
We do not sell your information, and we do not share it with anyone other than the service providers needed to operate this website and deliver the notification email.
Please Do Not Send Medical Details
A website contact form is not a secure channel for health information. Do not use it to describe symptoms, medical history or medications. Call the office instead, or bring it to your appointment.
